Making, changing or cancelling appointments

How do you schedule an appointment? Can you cancel if something unexpected comes up? We've got all the answers for you.
There are three ways to schedule your appointment:

Preparation

To ensure your appointment goes smoothly, you need to bring a few important items with you. Here you can find some helpful checklists:

    1. Always bring your eID, KidsID, or ISI+ card with you to the hospital.

    This is important, as it allows us to apply the third-party payer system. With this system, you only pay your portion of the costs (the co-payment), while the rest is directly covered by your Belgian health insurance provider.

    1. Please bring the following contact details
    • Your own phone or mobile number
    • The name and address of your GP
    • The name and address of the referring doctor
    1. Overview of medications you are taking, including the dosage and time of administration.
    2. Indication of any allergies.
    3. Medical documents, such as referral letter from your GP or referring doctor, X-rays, lab results, vaccination card, blood type card, etc.

In the following cases, you will need to provide additional information:

  • In the case of a work-related accident
    • Name and address of your employer
    • Name and address of your insurance company
    • Date of the accident
    • Claim number (if known)
    • Policy number
  • For an expert evaluation, you will also need to bring the referral letter from the organisation requesting the evaluation.
  • If applicable, bring a guarantee agreement from a Public Social Welfare Centre (CPAS/OCMW) or a payment commitment from another institution responsible for covering the costs.
  • For patients residing abroad:
    - Insurance card, form E112, or a guarantee statement from your health insurer
    - European Health Insurance Card (EHIC)

Register at the registration kiosk

  1. Always bring your eID, KidsID or ISI+ card with you to the UZA.
  2. Upon arrival, please first check in at one of the registration kiosks located in the central entrance hall or in the entrance hall of the Mother and Child Centre.
  3. Answer all the questions at the kiosk and take your ticket.
  4. Your ticket will provide you with further instructions:
  • Either proceed directly to the department where you have an appointment,
  • or visit the central desk to complete your registration.

Keep your ticket with you at all times during your visit at the UZA.

  • For administrative and medical purposes, it is essential that your information is fully and accurately recorded. Therefore, we will verify your details using your electronic identity card (eID), Kids-ID, or ISI+ card, which contain your official name and address. Additionally, you will be asked to provide your telephone number and the name of your general practitioner.

  • In some cases you may need to pay a deposit (advance) of €50:

    • If you are not affiliated with, or in compliance with, a Belgian health insurance fund.
    • If you do not have a registered address in Belgium or abroad.
    • If an OCMW or another institution is responsible for payment of the costs, but you cannot provide a valid payment guarantee
    • If you have overdue unpaid invoices.
  • During your consultation, our healthcare providers will regularly ask for your full name and date of birth. This is necessary to ensure safe, high-quality care. Correct patient identification is crucial, as our staff must know exactly who you are to provide the right treatment. If they do not ask for this information before a procedure or examination, please feel free to remind them.
